We realize that every person’s financial situation is different. For this reason, we have worked hard to provide a variety of payment options to help you receive the dental care you need and deserve that allows you to enjoy a healthy, beautiful smile with respect to your budget. Dental treatment is an excellent investment in an individual’s medical and psychological care. We are always available to answer your questions or assist you in any way we can.

To maintain the practice operations and prevent potential misunderstandings, we ask patients to accept and adhere to the following financial arrangements regarding their dental treatment.

Full Payment Cash Discount: We offer a 5% accounting courtesy for all treatment exceeding $500.00 that is paid in full (including expected insurance portion) with cash or credit card at the time of service. We will still file your insurance and the payment will go directly to the subscriber.

Major Services – Two Payment Option: We offer a two payment option for crowns, bridges, and denture treatment. We ask that you pay one half of your co-payment at the first appointment and the second half at the set date appointment.

Credit Card Payment Option: We allow (with a signed agreement form and established payment history with our office) a Credit Card Payment Option, which allows you to make three equal installments by credit card. One third of the payment is due at the first appointment, one third is due thirty days later, and the remaining third is due sixty days from the initial appointment. Our office personnel will charge these payments to your credit card on the due dates.

CareCredit Term Loan: We support CareCredit financing in offce. CareCredit is a special healthcare only credit card that you can use to break up that payment into more reasonable monthly payments. We’ll even take care of your interest! What’s more, you can use the card for other healthcare providers, or even the vet! Stil have questions? Regarding Insurance

We currently accept the following insurance:

Aetna, Anthem Blue Cross Blue Shield, Blue Cross, Blue Cross Blue Shield , Blue Shield, CareFirst Blue Cross Blue Shield, Cigna, Delta Dental, GEHA Dental Plans, Guardian, Humana Dental, Lincoln Financial Group, MetLife, United Concordia, UnitedHealthcare

We may accept assignment of insurance benefits, however the balance is your responsibility whether your insurance company pays or not. We cannot bill your insurance company unless you give us your complete insurance information.

Your insurance policy is a contract between you and your insurance company. We are not a party to that contract. If your insurance company has not payed on your claim within 45 days, the full balance will automatically be transferred to you. That balance will be due upon billing.

As part of the financial arrangement process, we will estimate what your insurance company will pay. We very much appreciate payment of your uninsured portion upon receipt of services. In the event that your insurance company denies payment of a service, you are responsible for that fee. Any unpaid balance after insuracnce pays is due within 30 days.

No Insurance? No Problem!

Maybe you’re in between plans for a short while. Maybe your employer doesn’t provide dental. Maybe your plan is HMO but you want to come see us anyway. Maybe you don’t like to buy into that insurance stuff. Maybe your plan is dental in name only and doesn’t actually cover anything (It happens. Seriously.)

Whatever the reason, for all intents and purposes, you’ve got no insurance. That’s okay. We have you covered.

We’d like you to meet our friend the Bogacki Dental Plan. This plan is an in-house alternative to insurance or the lack thereof. The plan, which features none of the political postulates that many insurances encumber you with, is quick and easy to sign up for and saves you money whether you’re just coming for typical cleanings or more intensive procedures.

With our plan you won’t need to worry about:

Deductibles: Since our plan is in-house, you don’t need to worry about insurance companies imposing deductibles on you. No matter how much or how little your procedures cost, our plan will offer you the same amount of coverage, whether it’s your first or fifth appointment.

Annual Maximums: Our plan has no cap on the amount of treatment it will cover in one year. If you have to get fifteen crowns done (God forbid), you’ll have the same coverage for all of them. We don’t cut out on you.

Missing Tooth Clauses: Your oral health is paramount to us. If you need some help and want to buy our plan just to get an implant or extraction done, we’re fine with that. Our plan covers all teeth, whether they’re present or not.

Waiting Periods: We’re not going to make you wait to get treatment done. you’re eligible for any and all treatment the second you sign up for the BDP.
