Information to prepare for your first visit:

• An insurance card and a form of ID, if applicable.
• A list of medications you are currently taking.
• New patient paperwork, which you are able to download here, or fill out in-office.
• If you’ve seen a dentist in the past six to twelve months, please bring in your most recent x-rays.

Remember! Your first appointment may be an hour and a half long.

Depending on the situation, you may given a treatment plan, which highlights certain procedures we recommend you have done for your continued oral health.

Note: All patients under the age of 18 must be accompanied by a parent or guardian at the appointment.

Cancellation Policy

Our office requires 24 hour notice to reschedule or cancel an appointment. While we do try to be understanding in terms of emergencies, we do expect patients to respect our time as much as we strive to respect theirs.
